PERIMENOPAUSE: THE INVISIBLE CHAOS MOST DOCTORS AREN’T TALKING ABOUT

Updated: Jun 19

Just in case this needs clarifying: perimenopause is not menopause. It’s the hormonal hurricane before menopause, and it can last 8 to 10 years. Yep, years. And yet, if you walk into your average doctor's office complaining of mood swings, night sweats, anxiety, or suddenly hating everyone… you’re likely to be handed a prescription for antidepressants or told it’s “just stress” or “normal aging.”

Here’s a very hard truth: Perimenopause is wildly under-researched, underdiagnosed, and misunderstood, especially in Canada. Most medical training around women’s hormones focuses on fertility and full menopause. What happens in between? Crickets.

WHAT IS PERIMENOPAUSE?

Perimenopause is the transitional phase before menopause, when estrogen and progesterone start to fluctuate wildly. This isn’t a gentle hormonal slide, it’s a biochemical rollercoaster with zero safety harnesses. It typically starts in your late 30s to mid-40s, though it can sneak in earlier.

THE SYMPTOMS: IT’S NOT JUST HOT FLASHES

Perimenopause can affect every single system in your body, not just your period. Here's what you might experience:

Hormonal Hell:

  • Irregular periods (shorter, longer, heavier, or suddenly MIA)

  • Mid-cycle bleeding or spotting

  • Shorter luteal phase (hello PMS x 1000)

  • Sudden changes in cycle length month to month

Mental & Emotional Whiplash:

  • Anxiety (especially new or worsened)

  • Depression or mood swings

  • Rage and irritability (Perimenopausal Rage is a thing)

  • Brain fog

  • Memory issues

  • Low motivation or apathy

  • Feelings of overwhelm at things that used to feel fine

Sleep Disturbances:

  • Waking at 2-3am and can’t fall back asleep

  • Night sweats

  • Restless legs

  • Insomnia despite exhaustion

Physical Mayhem:

  • Weight gain (especially around the midsection)

  • Breast tenderness

  • Joint pain

  • Fatigue that no nap fixes

  • Headaches or migraines

  • Palpitations or racing heart

  • Dizziness or lightheadedness

  • Increased food sensitivities or histamine intolerance

  • Blood sugar swings and new carb cravings

  • Acne (yes, again)

  • Hair thinning or loss

  • Dry eyes or mouth

  • Itchy skin (even without a rash)

  • Burning tongue or gums

Lesser-Known But Real AF:

  • Heightened sensory sensitivity (to noise, light, touch)

  • Electric shock sensations

  • Panic attacks out of nowhere

  • Bladder urgency or frequency

  • Vaginal dryness or atrophy

  • Changes in libido

  • Tinnitus (ringing in ears)

  • Feeling “wired but tired”

You’re not crazy. Your hormones are just throwing an uninvited rave in your endocrine system.

WHY MOST DOCTORS MISS IT

  • Hormonal tests are rarely run unless you push for them (and most GPs don’t know what to look for)

  • Testing fluctuating hormones with a single blood test? Useless. You need patterns.

  • Many doctors still believe perimenopause starts after your period starts changing, but symptoms often begin years before that

  • Women’s health has been underfunded, under-researched, and brushed off as "psychosomatic" for decades

WHAT CAN HELP (SUPPLEMENTS & LIFESTYLE)

Supplements often used to support perimenopause:

  • Magnesium glycinate  for sleep, anxiety, blood sugar

  • B6 & B-Complex  for mood and hormone production

  • Ashwagandha  adaptogen for cortisol regulation

  • Chaste Tree (Vitex)  may support progesterone in early perimenopause

  • Omega-3s  anti-inflammatory, hormone-friendly fats

  • DIM & Calcium-D Glucarate  estrogen metabolism support

  • Bioidentical Progesterone Cream  prescribed or over-the-counter support for those deficient

  • L-Theanine & GABA  nervous system support

  • Iron + Ferritin support  especially with heavy or frequent periods

Always work with a practitioner as some herbs and supplements can interact with medications or worsen symptoms if mistimed.

Perimenopause is not "just menopause lite." It’s a full-blown hormonal upheaval that deserves proper diagnosis, compassionate care, and real support.

You're not making it up. You're not weak. You’re not broken.

You’re going through a biological transformation in a world that hasn’t caught up. But now? You know more, even though it's infuriating to have to advocate for yourself when this affects 50% of the population. Time to demand better, friends. I can genuinely say my GP hates me because I demand they know more.
